 I've got several hundred rounds through both of mine and zero slam fires

A slam fire is something you do on purpose. The Ithica 37 shotgun was made to do this. You rack a round and fire it, then as you continue to hold down the trigger, you rack another round and it fires. I saw a video of someone doing that with the DP 12, but the DP 12 will only fire the right barrel when doing the slam fire.
